Flu Vaccine Clinic

Flu clinic for Lakeview students, parents, and employees. Mist and injection will both be available. Flu shots are $25 and Flu Mist is $35.  We take Master Card and Visa, cash and checks which can be made to Health Check Services.  We can also provide receipts if needed. 

Vaccine consent forms are available in each division office and also in the infirmary or click here to download the vaccine consent form.
 
We will not have a sign up sheet; come by at your convenience.
