Flu Vaccine Clinic

Flu clinic for Lakeview students, parents, and employees. Mist and injection will both be available. Flu shots are $25 and Flu Mist is $35.  At the clinic, you can use Master Card, Visa, cash, and checks which are to be made payable to Health Check Services.  They can also provide receipts if needed for insurance claims. 

Vaccine consent forms are available in each division office and also in the infirmary or click here to download the vaccine consent form.
