Free GHSA Physicals

Rising 6-12 grade students who participate in athletics are required to have a valid GHSA physical form on file at school. This is an opportunity to get a free physical at school that will be valid for the entire 2015/2016 school year!  Click here to get a copy of the form.  Parents or students fill out page one.  Both parent and student sign at the bottom of the first page.  Bring this with you May 9 for your physical.  Make your appointment time by emailing Coach Little and jb.little@lakeviewacademy.org
