Students,
If you are planning to bring a guest/date to Prom who attends another school, you must complete THIS FORM in advance, including their parent/guardian's contact information, for security and safety purposes. This form should be submitted no later than Thursday, April 309.
Parents,
The senior walk-out at Prom begins at 8:00 PM, so all students should arrive no later than 7:45 PM. Prom is scheduled to last until 10:30 PM. To help keep your students safe and accounted for, we will not allow any underclassmen (grades 9-11) to leave Prom before 9:30 PM unless you email your permission for them to do so to matt.phillips@lakeviewacademy.org.
Lakeview Academy is a private, coeducational day school for students in preschool through 12th grade, located in Gainesville, GA minutes off I-985/Hwy 365.
