School Forms & Health Records
Required school forms including health information, emergency contacts, transportation, and family information updates are now available on the Lion Portal. Medical forms must be completed by August 1, 2024, and all other school forms are due by August 10, 2024.
Students requiring an allergy, asthma, diabetes, or seizure action plan can find them on the School Nurse page in the Lion Portal. To discuss any medical needs or concerns for your child, the school nurse is available via email at scarlet.pendarvis@lakeviewacademy.org.

For student medical/health information, Lakeview uses a secure and HIPAA-compliant online database called Magnus Health which is accessed via a link on the Resource Board of the Lion Portal. Instructions and details for accessing and using Magnus are linked here: Magnus Health Record Instructions. So that the school nurse has time to review the information before classes begin, Magnus Health information is due by August 1, 2024.
