Free Athletic Physicals

Rising 6-12 grade students who participate in athletics are required to have a valid GIAA physical form on file at school. 
 
Click here (link coming soon) to download the physical form. Students should bring the physical form with them and the top half of the first page of the form must be completed prior to the physical.
Submitting your physical form:
Completed physicals are to be uploaded to Magnus Health via the Lion Portal beginning in mid-June.
