• PreK 3, PreK 4 and Kindergarten

  • Students must be at least three years old at the start of the school year for which they are applying.
  • During the application process, provide a copy of the student's birth certificate, immunization certificate, and social security card, submit a Background Information Sheet (to be completed by the parent), and request the child's current teacher to complete the online Preschool Teacher Recommendation form.
  • Copies of IEPs, special services, or psychological reports, if any.

  
  Please submit the $85 application fee using the links on the online application. After you complete the application form, you will receive an email acknowledgment of your application to Lakeview Academy. 
  
  You can check the status of your application by logging back into the site using the username and password you created when you applied. Once on this site, click the clipboard icon to check outstanding documents.
  
  After the above-requested documents are received, we will contact you regarding admission acceptance.

• 1st - 12th Grade

  • Parents sign the Transcript Release Form, which can be found in the Lion Portal, and give it to the applicant’s current school.
  • The complete application must include:
    • birth certificate, immunization certificate, and social security card
    • academic grades from the current period and three previous years
    • standardized test scores for three previous years, if applicable
    • attendance records
    • conduct report (if any)
    • Completed teacher recommendation forms
    • completed assessment for 2nd-8th grade students
  • Copies of IEPs, special services, or psychological reports, if any.

  
  Please submit the $85 application fee using the links on the online application. After you complete the application form, you will receive an email acknowledgment of your application to Lakeview Academy. 
  
  The Admission Office will contact you to schedule a suggested day visit with a student host after all required documents are received. The applicant will join a student host in their current grade for classes, lunch, and a tour. This provides applicants an opportunity to experience Lakeview from a student’s perspective.
  
  After the above-requested documents are received and your child has completed their visit, we will contact you regarding admission acceptance.
