Athletic Information and Forms

Typically, 79% of middle school students and 80% of upper school students participated in at least one sport. This page provides information about the sports offerings at Lakeview Academy, including participation eligibility, the season for each sport, and contact details for the coaches.

Lakeview Academy competes in the Georgia Independent Athletic Association (GIAA), an ancillary division of the Georgia Independent School Association (GISA).

Free Sports Physicals May 11, 2026

Mark your calendar! We are offering free sports physicals on Monday, May 11, from 3:30-5:30 pm in the WAC to Lakeview students in grades 5-11 who may play a school sport next year and to lower school students playing youth tackle football. Athletes must have a completed preparticipation physical on file to participate in a MS, JV, or varsity sport. This is the only physical clinic offered at Lakeview for the 2026-2027 school year. Physicals are good for one year from the date of the physical.

To give permission for your child to participate in the free physical, and so the physician will have your child's health information, download and complete the Participation Physical Form for May 11, 2026 form, and have your child bring it along with all of his/her other required and completed forms to the physical on May 11.

SUBMITTING ATHLETIC CLEARANCE FORMS
A completed physical evaluation and all required athletic forms will be returned to your child to bring home for you to upload to their Magnus Student Health Forms in the Lion Portal. See the instructions below.
 
Note: DO NOT TURN IN PAPER FORMS TO COACHES OR OTHER STAFF. THESE MUST BE UPLOADED AT HOME BY THE PARENT.

Required Athletic Forms

MS, JV, and Varsity athletes must have the GISA Pre-Participation physical and signed athletic clearance acknowledgment forms on file with athletics PRIOR to the start of seasonal practices for ALL sports. Physicals are good for one year from the date of the physical and the athletic clearance forms are good for the entire school year. Completed and fully signed forms are to be uploaded via the Magnus Health app.
