Important Health Information For The New School Year

Dear Lakeview Parents, 

One of the most important aspects of my job is ensuring a healthy and safe environment for your children when they return to school this August. Please review the following information regarding health related issues at Lakeview. More information can also be found on the School Nurse webpage. Medical Information
Prior to each school year, there are several required electronic records that must be completed for new students and updated by returning students. These online forms contain medical and allergy information, medication authorization, emergency contact information, permission for (field trip) transportation, bus transportation registration, grandparent data and Extended Learning registration. All of these forms are accessed via the Lion Portal.

If your child has had an updated immunization record within the last year, 
please provide the school with an updated copy prior to the start of school. You may turn them in to me in the Lower School Office, email it to me, or fax it to 770-536-6142.

Medical Action Plans
If your child has a medical issue such as asthma, seizures, diabetes or severe allergies, you will need to complete a medical action plan or provide one from your physician. These forms are available as downloads on the School Nurse page and are also located on our website under the A-Z Site Index (the link is located at the top of each web page) and should be updated yearly with any changes.
Food Allergies
Please be aware that at Lakeview we have students with food allergies throughout the school. The most common and typically most severe allergies are peanuts and tree nuts. It is our goal as a school to provide the safest environment possible for all of our students. You will be notified by your teacher of specific allergies within a classroom. Please be aware of this health issue and we highly recommend you avoid sending peanut/nut product to school in your child’s lunch or snack when possible. While we cannot provide a ‘peanut/nut free’ environment, increased awareness on your part will help reduce the possibility of a life threatening allergic reaction.
