Upper School Back-to-School Information
Required Athletic Forms
MS, JV, and Varsity athletes must have the GISA Pre-Participation physical and signed athletic clearance acknowledgment forms on file with athletics PRIOR to the start of seasonal practices for ALL sports. Click "Read More" for additional information.
Physicals are good for one year from the date of the physical and the athletic clearance forms are good for the entire school year. Completed and fully signed forms are to be uploaded via the Magnus Health app.
